the first day of employment. Part-time employees are not eligible.

The health plan claims administrator, Anthem, offers a support
team that provides assistance if you or a family member has a
chronic condition, such as the following:

• Cardiovascular Obstructive Pulmonary Disease (COPD).
• Coronary Artery Disease.
• Congestive Heart Failure.
• High-Risk Maternity.
When you join ConditionCare, you will get:
